How a Complete Sleep Study Works

From clinical assessment to diagnosis and follow-up

A sleep study records breathing and other body signals while you sleep. This guide explains the main stages, the difference between home testing and laboratory polysomnography, and why the results must be interpreted by a qualified healthcare professional.

1. The process starts with a clinical assessment

Symptoms such as loud snoring, witnessed pauses in breathing, morning headaches, unrefreshing sleep or excessive daytime sleepiness can have different causes. A healthcare professional reviews symptoms, medical history, medications and relevant risk factors before deciding which type of sleep test is appropriate.

Important: A screening questionnaire or consumer device cannot independently confirm or exclude sleep apnoea. Diagnosis should be based on clinical evaluation and an appropriate sleep study.

A portable monitor is fitted before bedtime and used in the patient’s normal sleeping environment. An overnight study in a sleep laboratory records a broader set of signals under the supervision of trained staff. It can include brain activity, eye movements, muscle activity, breathing, oxygen, heart rhythm, limb movements and video. It may be preferred when the clinical picture is complex or when another sleep disorder is suspected.

The more comprehensive test is not automatically the right test for every person. The choice depends on symptoms, medical history, the likelihood of obstructive sleep apnoea and the clinician’s judgement.

4. Preparing for the night of the test

Patients should follow the instructions provided by the sleep service. These may cover medication, alcohol or caffeine, skin preparation, sensor placement and usual bedtime routines. Sensors need to remain connected for enough recording time to produce interpretable data.

If a home test has an obvious technical problem or insufficient recording time, the test may need to be repeated. A negative or inconclusive home test does not always exclude a sleep disorder.

5. How the results are reviewed

The recording is checked for technical quality and then interpreted in its clinical context. For suspected obstructive sleep apnoea, the report may include the number of breathing events per hour, oxygen changes, event type, body-position effects and recording reliability.

A single number should not be interpreted in isolation. Symptoms, oxygen burden, sleep time, other medical conditions and test limitations can materially affect the clinical meaning of the result.

Next steps may include: no treatment, lifestyle or positional measures, further testing, CPAP therapy, an oral appliance, specialist assessment or another treatment pathway. The appropriate decision belongs to the treating clinician.
